BILLING INFORMATION

Medical bills can be confusing. We strive to help make this process easier for you. Our office will automatically submit claims directly to your insurance carriers. Insurance coverage can vary dramatically, depending on your policy. Check with your insurance provider if you have questions about your coverage. Please understand that you will be responsible for insurance co-pays, co-insurance and deductible amounts as well as for charges for non-covered services. Payment is expected at time of service (Download Financial Policy).
 
We are happy to provide cost estimates for any proposed services. Our billing staff is available to assist you with questions from 9am – 4pm.

PAYING YOUR BILL

Full payment is expected when you receive a bill requesting payment.  We accept VISA, MasterCard, Discover, American Express, payment by check or CareCredit. If you can only make a partial payment, please contact our office to set up a payment plan.  We may request that you complete a Statement of Income and Expenses (Download Statement of Income and Expenses) to support such requests.

Southwest Idaho Ear, Nose and Throat, PA is a Non-Participating provider with Medicare, therefore we do not routinely accept assignment on Medicare claims. We will file claims to Medicare, but on any unassigned claims, Medicare will send payment to the patient. Payment in full is expected at the time of service for Medicare unassigned claims.
